Latest Card or Form 137 (original and photocopy)

2 pcs. 1x1 ID picture

Registration Fee - Regular =Php 50

An Individual is qualified to take the PEPT If he/she completed the grade as shown below:

Grade Age at the time of registration

Grade 1 at least 8 yrs. old

Grade 2 at least 9 yrs. old

Grade 3 at least 10 yrs. old

Grade 4 at least 11 yrs. old

Grade 5 at least 12 yrs. old

Grade 6 at least 13 yrs. old

Duration: within the day
