Kindergarten and Elementary: Early Registration: every last Saturday of January

Who May Avail of the Service?

v Kindergarten age - 5 years old as of Oct 31 of the current year

v Elementary age - 6 -11years old

Requirements for Enrollee:

1.Kindergarten and Grade I

v Birth Certificate (original and xerox copy)

2.Elementary – Grade II –VI

v Report Card (Form 138)

3.Transferees

v Report Card (Form 138) with LRN

v Copy of Bith Certificate

v Form 137-E with LRN

v Certificate of Enrollment (In case of inavailability of Form138)

Duration: half day
